Optimal Physical Therapy in Lake Mills is holding a free clinic on Friday, July 26 from 8:00-noon for sports physicals. Appointment required - call 920-648-2400 to reserve your spot - no walk-ins.
If parent unable to attend, child must bring the Medical Eligibility Form that has been signed by a parent/guardian. Well visits or vaccinations are NOT included or available during this event.

MMS Athletic opportunities for 7th and 8th grade students:
- Cross Country
- Girls Volleyball
- Boys Basketball
- Girls Basketball
- Wrestling
- Track and Field
6th graders may participate in Cross Country and Wrestling.
Forms & Fees
WIAA required physical form, Concussion Form, Athletic Code of Conduct and Emergency forms are required for all athletes in order to participate in games and practices. Physicals are good for 2 school years from the date of the exam and a form is filled out by the doctor and kept on file in the office. Required forms can be turned into the middle school office, the coach or faxed to the district office at (attn: middle school) FAX # 608-655-4481.
